Students should always consult with and obtain their committee chair's approval before beginning the substitution or waiver process. A Substitution/Waiver Form may be obtained from the Graduate Program Office, Ellison Hall 2834 or from the link above. After the Substitution/Waiver form is complete, return the form back to the Graduate Program Office, to be placed in the student’s file.
When your proposal is approved, your doctoral committee must sign the Approval of Dissertation Proposal Form. This form is returned to the Graduate Program Office, Ellison Hall , room 2834 and placed in your student file.
